Reconstructive Foot and Ankle Surgical Associates was founded by Dr. Jose R. Ruiz in 1997. To provide a better service to all patients of Millville and other communities in the Southern areas, the office relocated its facilities to 1600 North High Street in Millville, NJ (formerly 1306 North High Street) with a larger state of the art facility, 6 operative rooms and ample parking space and easy access for patients with disabilities. The practice specializes in and is limited to diseases and surgery of the foot and ankle and guarantees both convenient office hours and the best personal care available.

Co-Pays and Deductibles
Payments for services are the responsibility of the patient. All copays and deductibles are due at the time of the visit. Many patients have health insurance and rely on their coverage to pay the major portion of their health care costs. It is important for you to understand that health insurance is a contract between you “the patient” and the insurance company. We are not a party of that contract. Therefore, it is a good idea for you to become familiar with you own insurance coverage. It is important to remember that whatever portion of services not paid by your insurance is your responsibility. Questions on insurance matters should be discussed with the Medical Biller, who is experienced in handling insurance claims.
Referrals and Authorizations
Unless your insurance allows you to go directly to a specialist, you must obtain a referral from your primary care physician, prior to receiving treatment in our office. It is your responsibility to coordinate in a timely fashion, so that you can bring the referral at the time of the visit.
